The emergency department of a hospital is fully staffed and equipped to handle life-threatening emergencies. My Doc Urgent Care in Garfield, New Jersey, does not treat life-threatening emergencies.

We want your visit with us to be as convenient as possible. Please be sure to bring the following:
- Driver’s license or photo ID
- Insurance information and co-pay payment – or – cash/credit card for self-pay patients
- List of all current medications, allergies, and past surgeries
- Parental authorization form (for minors appearing without a parent or guardian)
If you’re here for an employer physical*, please bring the following with you:
- All items on the “for patient” list
- Completed Employer Authorization Form
- Any documents needed for a DOT physical
